Job Description (Overview)
- Complete initial and ongoing assessments of clients.
- Develop, approve, and update individualized care plans.
- Supervise and evaluate aides and LPNs.
- Provide direct nursing care when necessary.
The LPN works under the supervision of the RN, delivering delegated nursing tasks, supervising aides, and ensuring care is provided according to each client’s service plan.
Job Description (Overview)
- Perform nursing tasks as delegated by the RN.
- Support aides and provide supervisory visits as directed.
- Assist with service plan updates and follow-up visits.
- Document services provided and report changes in client condition.
PCA/HHA
Homemakers
RNs/LPNs
PCAs/HHAs provide hands-on assistance with daily living, light homemaking, and companionship, helping clients remain independent at home.
Job Description (Overview)
- Assist clients with activities of daily living.
- Provide light homemaking.
- Run errands, outside of the individual, such as grocery shopping or medication pickup.
- Offer medication reminders.
- Contribute and participate in the execution of the Plan of Care.

- Perform light housekeeping.
- Prepare simple meals and snacks.
- Run errands outside the home.

- Report unsafe conditions in the home to the office.
